This Privacy Policy applies to information collected through the Website and related online features (for example, online forms, telehealth access links, and portal access points).

Toetal Health Solutions also maintains a separate HIPAA Notice of Privacy Practices ("NPP") that describes how we use and disclose Protected Health Information ("PHI") in our capacity as a healthcare provider. That NPP governs our uses and disclosures of PHI, and if there is any conflict between this Privacy Policy and the NPP with respect to PHI, the NPP will control.

We may collect the following categories of information when you use the Website:

• Non-Personally-Identifying Information: Browser type, device type, operating system, language preferences, referring and exit pages, date and time of visits, general location inferred from IP address, and similar technical data.

• Appointment, Telehealth, and Portal Information: Information you submit when you request an appointment, use telehealth, or access the patient portal (for example, preferred dates, reasons for visit, or contact information). When this information constitutes PHI, it is handled in accordance with HIPAA and our NPP.
• Communications Information: Content of messages you send to us (for example, via contact forms or email) and your preferences for SMS, email, and voice reminders.

We collect this information directly from you, automatically through your use of the Website (including cookies, as described below), and from our service providers such as Athenahealth when you use their connected services.

• Affiliated Entities: Among Toetal Health Solutions Management LLC, Toetal Health Solutions NV P.C., Toetal Health Solutions CA P.C., and other affiliated entities for operational, administrative, and care coordination purposes.

We do not sell or rent your personal information, and we do not disclose email addresses or other contact information to parties not directly involved in patient care or the general operations of our practices.

Over the past 12 months, we may have collected the categories of personal information described in Section 3 above from California residents for the business purposes described in Section 6. We may disclose these categories of information to the types of third parties described in Section 8 for business purposes such as website operation, security, and provision of services.

We do not sell personal information, and therefore we are not required to provide a "Do Not Sell or Share My Personal Information" link. California residents may contact us using the information in Section 15 to ask questions about our privacy practices.

These California disclosures are separate from the rights that apply to PHI under HIPAA, which are described in our HIPAA Notice of Privacy Practices.

14. Do Not Track

Some web browsers include a "Do Not Track" ("DNT") signal that can indicate a user's preference regarding online tracking. The Website does not currently respond to DNT signals. You may use your browser settings and other tools to control cookies and similar technologies as described in Section 4.
